Abstract In every hospital, Casualty department deals with the emergency cases. These emergency cases also includes medicolegal cases like road traffic accidents, Poisoning, assaults are dealt in the casualty and detailed MLC report is made here after giving the proper primary treatment and life saving measures. The police is informed about MLC as early as possible. Important preventive measures like drawing public attention and awareness towards traumatic casualties can help in the prevention or management of unnatural (medicolegal cases). Considering the importance of above points, a one year retrospective study from 1st March 2018 to 31st March 2019 was conducted in the Casualty department of Tertiary Care Centre.

In an increasingly globalised world, migratory movements is continuously shaping the countries all over the world. Some countries like India and Ireland, which set the example of economic development and social integration, have the positive impact of the migration by globalisation and some countries like USA, which recently witness racism, xenophobia and discrimination have the negative impact on the migrants. It does not mean India do not face fragmentation and USA do not have cohesion. USA have many stories which show successful integration process, that facilitated the lives of immigrant communities, but being a developed country it still suffers from cultural alienation. In these countries, borders are built within borders to create cultural divides that do not allow people to integrate. Recently, this problem has become more prominent due to the rise of terrorism, clash of cultures in the world, leading to the glorification of stereotypes. People are becoming less accepting towards anyone who does not belong to their region. Migration does not stop after people move from one place to another place. The main question start after that ‘now what’ they will do. That is why this topic needs to be discussed thoroughly in order to find better solutions. This paper will begin with an analysis of different approaches to Migration, discuss the target groups for integration policies, provide indicators of the current situation of migrants and proceed to an analysis of integration tools: legislation, social policies and participatory processes. It will focus not only on the impact of migration but also on social integration, mix culture like indo-western culture in a comparative basis.

In this paper, investigations are made to analyze the human body temperature during wound healing process due to surgery. Wound is considered after the skin graft. Skin graft is a technique used in plastic surgery. Skin is the first line of defense between the human and environment, it is very susceptible to damage. Internal body or core temperature (Tb) is one of the clinical vital signs along with pulse and respiratory rates. Any disturbance in body temperature will drive complexities in wound healing process. These studies are important in the mechanism of establishing the limits of thermal regulation of human body during the healing process in different situations and conditions. The Finite element method is used to analyze tissues temperature for normal tissues (donor site) and abnormal tissues (tissues after surgery). Appropriate boundary conditions have been framed. Numerical results are obtained using Crank Nicolson Method.
Physically unclonable function (PUF) is a hardware security module preferred for hardware feature based random number and secret key generation. Security of a cryptographic system relies on the quality of the challenge-response pair, it is necessary that the key generation mechanism must unpredictable and its response should constant under different operating condition. Metastable state in CMOS latch is undesirable since it response becomes unpredictable, this feature used in this work to generate a unique response. A feedback mechanism is developed which forces the latch into the metastable region; after metastable state, latch settle to high or state depends on circuit internal condition and noise which cannot be predicted. Obtained inter hamming variation for 8 PUF is 51% and average intra hamming distance is 99.76% with supply voltage variation and 96.22% with temperature variation.

Initially it was not much rigid but gradually people belonging to upper castes for their own selfish means to maintain their monopoly made this arrangement hereditary and started treating people of lower castes disgracefully. For preservation of this system, people started controlling their women to prevent inter-caste marriages and the concept of endogamy came up. This robbed away many types of freedom from women. For women belonging to lower castes, this situation is worse as they are doubly subjugated on the basis on caste as well as gender. Men belonging to their own caste treat them as secondary beings. This paper throws light on this intersection. How intersection of these two kinds of inequalities place them at the lowest position in Indian society. Dr. B.R. Ambedkar rises as their leader who all his life worked for empowerment of downtrodden section of society. He argues that education is the primary tool for evading these differences among people. He further emphasizes to adopt the concept of exogamy to break the backbone of Indian caste system and to immediately leave a religion or culture which legitimizes such system of inequality among people of the same land.

Background: Hypertension (HTN) is defined as a blood pressure (BP) ≥140/90 mm Hg. The prevalence and mortality from HTN, globally and locally, have been on the increase since the 1980s. This study aimed to detect young adults (18-40 years) in Enugu State who are hypertensive, in order to raise the awareness that HTN is also common in young adults. Its early diagnosis and management could help to reverse the trend in the number of cases of HTN in Nigeria which had been predicted could hit 39.1 million by 2030 if the present trend is allowed to continue. Methods: Participants were administered with structured questionnaire. Their BPs, heights and weights were measured using sphygmomanometer, stadiometer and scale balance respectively. Data were analyzed as proportions, Pearson product moment correlations and Chi square tests using MaxStat (version 3.60) statistical software. Results: The overall prevalence of HTN was 21.3%; 20.7% in the 18-29 years age group; 25.9% in the 30-40 years age group; 24.3%, in males; and 20.3% in females. Pearson product moment correlation coefficient r between age and HTN=0.462, between sex and HTN=0.328, between family history and HTN = 0.513, between salt intake and HTN=0.613, between BMI and HTN=0.152, between physical activity and HTN=0.293, between smoking and HTN=0.308. Conclusions: HTN in young adults is more common than previously thought and if unmanaged in early life usually progresses to HTN later in life.
A medication error is any preventable event that may cause or lead to inappropriate use of drugs or patient harm if it is not under the control of the health care professional. Despite the fact that healthcare professionals take the responsibility very seriously, it has been estimated that 250 000 Americans die annually due to medication errors. Although, estimates show that one in ten patients worldwide is affected by medication errors while receiving hospital care, the harm can be caused by a range of incidents or adverse events. The exact incidence of medication errors in the neonatal intensive care unit (NICU) is unknown. In neonates especially premature infants, the immaturity of developing body systems affects the absorption, distribution, metabolism and excretion of drugs and therefore an exponential risk for medication errors is present. The risk of medication errors in children are much greater than adults.

Contact between pharmaceutical company representatives (PCRs) and medical practitioners is viewed by drug companies as a vital part of their marketing strategy. Studies show that PCRs employ persuasion disguised as information. Because PCRs are keenly involved in competitive marketing, they are more likely to use persuasion than information during their promotional visits. Information increases customer awareness of a particular product without influencing the preferences. Persuasive marketing directly influences a consumer’s brand preference. Positively influencing a prescriber’s attitude towards a product is essential for effective marketing since there is an association between attitude, intention and behaviour. Numerous studies find that pharmaceutical promotional interactions have an impact on physician’s perceptions and their prescribing practices. Yet, many of today's health professionals do not acknowledge their vulnerability to being misled by pharmaceutical marketing. This study demonstrate that Pharmaceutical promotion strategies do not differ greatly from those of other types of marketing, PCRs more likely to use persuasion to highlight the perceived difference of their particular brand. Persuasion often has a subtle influence that is not easy to detect and physicians should receive education regarding the drug promotion techniques used by pharmaceutical companies. Persuasion often has a subtle influence that is not easy to detect. Overconfidence is a major risk factor for being misled. This analysis highlights some critical aspects of persuasive communication by PCRs in promotional visits. It will help prescribers to interpret, evaluate and respond appropriately to manipulative behaviour by PCRs.

Vitamin C is associated with history of the cause of the ancient hemorrhagic disease scurvy. Vitamin C is an essential nutrient with important antioxidant properties. It is required by the body for normal physiological function. The body cannot synthesize vitamin C, it is present in nature through foods and other natural sources and it exists as a nutritional food supplement. The antioxidant activity of vitamin C protects the body from free radical damage. Vitamin C is essential for the development and maintenance of connective tissues. It is used as therapeutic agent in many diseases and disorders. Vitamin C plays an important role in several metabolic functions, as the conversion of the amino acid, tryptophan, to the neurotransmitter, serotonin, and the conversion of cholesterol to bile acids. Vitamin C supplementation resulted in a significant increase in vitamin C levels in populations; its high intake is associated with positive effects on cardiovascular risk factors. Vitamin C protects the immune system, reduces the severity of allergic reactions and helps to fight infections. It has an important role in bone formation, wound healing and the maintenance of healthy gums. There is profound beneficial effect of vitamin C in respect to human diseases as cancer, atherosclerosis, diabetes, neurodegenerative disease and many metal toxicities. Several vitamin C analogs have been produced as anticancer and antioxidant activity. Vitamin C is useful if it is used as adjuvant therapy for several chronic diseases. Thus, this review summarizes the importance of vitamin C in the body’s physiology and biochemistry, in addition, the different mechanisms that vitamin C is implicated to treat different acute and chronic diseases. Future exploration should pay attention to chronic disease management by vitamin C.

In the present study the effect of the ethanol extract of Culcasia falcifolia on the neurotransmitter levels were evaluated. Pentylenetetrazole (PTZ) (60 mg/kg body weight, i. p) was administered to all the groups of mice to induce convulsions. After observing the convulsion, all groups rats were sacrificed, whole brain was dissected out and separated the forebrain. The brain tissue was homogenized and then centrifuged. The aqueous phase was taken for the assay. The reading was obtained using spectroflurimeter. The result of the present study shows that the ethanol extract of Culcasia falcifolia at the dose of 200 and 400 mg/kg body weight significantly increased the level of inhibitory neurotransmitter GABA and also showed significant increase in the levels of dopamine, noradrenalin and serotonin when compared to seizure control group (PTZ treated group) and was almost equivalent to control group. The findings of the study showed that the extract at 200 and 400 mg/kg body weight significantly (p<0.001) increased the levels of GABA, dopamine, serotonin and noradrenaline. The ethanol extract of Culcasia falcifolia exerted antiepileptic activity by decreasing seizure susceptibility to pentylenetetrazole-induced seizure in mice due to restoration of GABA, serotonin, dopamine and noradrenaline concentration levels in mice brain.
Background: Exposure to cigarette smoke has been associated with pulmonary and reproductive dysfunctions; inflammatory response, oxidative stress and oxidative DNA damage induced by polycyclic aromatic hydrocarbons (PAHs) present in cigarette smoke have been implicated in the pathogenesis of these disorders. The peak expiratory flow rate (PEFR), a biomarker of inflammation and oxidative DNA damage (8-hydroxy-2-deoxyguanosine (8-OHdG), tumor necrosis factor alpha (TNF-α)), reproductive hormones (testosterone (TST), luteinizing hormone (LH), follicle stimulating hormone (FSH)) cotinine and urinary PAH metabolite (1- hydroxypyrene (1-HOP)) were estimated in male active smokers. Methods: One hundred men aged 20-47 years, comprising 50 active male smokers and 50 non-smokers, were randomly recruited into this comparative cross-sectional study. The PEFR was measured using a peak flow meter, serum levels of cotinine, FSH, LH, TST, TNF-α, and urine 8-OHdG by enzyme-linked immunosorbent assay and 1-HOP by high-performance liquid chromatography. Data analysis was done using a t-test and correlation analysis at p≤0.05. Results: Smokers had significantly higher cotinine (49.73±31.76 versus 0.51±0.69 ng/ml, p≤0.001), 8-OHdG (16.34±12.10 versus 5.79±2.14 ng/ml, p≤0.001) and lower PEFR (309.20±56.05 versus 452.80±45.76 L/min, p≤0.001) and LH (5.75±2.06 versus 6.97±2.79 mIU/ml, p=0.015) compared to non-smokers. Duration of exposure to cigarette smoke correlated positively with cotinine (r=0.937, p≤0.001) and 1-HOP (r=0.813, p≤0.001) while cotinine correlated positively with 1-HOP (r=0.863, p≤0.001) only in smokers. Conclusion: Reduced lung function and luteinizing hormone and concurrent increase in oxidative DNA damage associated with exposure to cigarette smoke may suggest the involvement of PAH-induced DNA damage in the development of pulmonary and reproductive impairment in smokers.
ABSTRACT Background: One of the many functions of the liver is the synthesis of carrier proteins and hormone metabolism, and liver diseases have been linked to a variety of endocrine disorders. Aim:The study's goal was to look at the spectrum of chronic liver disease and the relationship between thyroid profile and severity of liver damage in a tertiary hospital. Methods and materials: The current study was a single-center, tertiary care hospital-based,case-control study that included 50 cases of liver cirrhosis/chronic liver disease and 50 age/sex matched healthy controls (chosen at random from relatives attending OPD with patients). Thyroid function tests were performed and results were compared between cases and controls.Result : This study included 50 cases of liver cirrhosis/chronic liver disease and 50 healthy controls.The mean age and gender of cases and controls were comparable, and the difference was not statistically significant. The majority of cases (76%) had alcoholic liver cirrhosis, with the remainder having non-alcoholic liver cirrhosis (16%) and chronic viral hepatitis (8%). As per Child-Pugh Score, the majority of cases (40%), followed by Child-Pugh C (32%) and Child-Pugh A (28%). In the current study, free T3, free T4, and TSH levels were compared between cases and controls, and abnormal values were found in cases, indicating a statistically significant difference. Serum thyroid profile abnormalities were observed as Child-Pugh Score Classes advanced, and the difference was statistically significant for free T3 and free T4. Conclusion: As compared to healthy subjects, patients with liver cirrhosis had abnormally high levels of circulating thyroid hormone, and severe abnormalitieswere linked to an advanced Child Pugh score. Derangement in thyroid profile is common in patients with cirrhosis of liver. Low free T3 and T4 levels are associated with more severe liver injury and may be used for prognostication in patients with cirrhosis of liver. This review paper aims to provide a comprehensive overview of the nutritional composition, Pharmacological activities, health benefits, and potential applications of Macrotyloma uniflorum. Horsegram has a high amount of protein, dietary fibre, and a variety of vital minerals and vitamins that make up its nutritional profile. Horsegram also has a high content of antioxidants, flavonoids, and phenolic compounds, all of which contribute to its potential health advantages. Horsegram intake has the potential to help manage a number of chronic conditions, including diabetes, obesity, cardiovascular problems, and gastrointestinal disorders, according to many studies. The review investigates the possible health advantages of Macrotyloma uniflorum, emphasising its function in controlling blood sugar, managing weight, lowering cholesterol, and antioxidant activity. Horse gram's medicinal potential is further increased by the presence of bioactive substances that include anti-inflammatory, antibacterial, diuretic, antiobesity, analgesic, anaphylactic action, and anticancer activities. In conclusion, this review paper presents a comprehensive evaluation of Macrotyloma uniflorum, highlighting its nutritional composition, health benefits, and potential applications. The findings underscore the significance of horse gram as a valuable functional food ingredient and encourage further additional investigation of its therapeutic properties for human health and well-being.
Abstract Background. Numerous studies have found that various core muscles are involved in subjects with chronic low back pain. The evidence for profiles of respiratory muscle involvement in normal low backache subjects has not been reviewed explicitly for low back pain. Aims. The study’s primary objective was to scientifically analyze, condense, and to evaluate the evidence for the involvement of respiratory muscles in subjects suffering from chronic low back pain. Methods. This evaluation procedure will trail the guidelines of PRISMA and MOOSE. The assessors will use a custombuilt tool to extract data from the studies, regardless of the methodology used. A modified Downs and Black index will be used to assess the superiority of the methods used in the selected studies. Results. This index evaluates the characteristics of the subjects, the timing of the outcomes, and the characteristics of the interventions, and quantitative, formal analysis, so-called meta-analysis, is performed if necessary. The study’s effect, including the evidence, will be determined using the GRADE a framework for a systematic approach. Earlier research has found that core muscles are involved, and the interventions are tailored to include deficiencies and intrusions that target specific muscle groups. Conclusion. As an additional component that can be targeted by clinical practitioners in their day-to-day practice and added to rehab programmes, this systematic review protocol will summarise the list of evidence available for the rehabilitation of low back ache and highlight the involvement of respiratory muscles in lowback ache subjects. A 55‑year‑old male patient who has undergone physiotherapy sessions on and off for about 6 months has been admitted to the hospital for the recurrence of back pain along with it he has slight breathing issues while performing day‑to‑day activities, especially in forward bending activity and has been treated with pain killers and muscle relaxants during the medication periods. The patient is feeling better again, but his symptoms continue to worsen. Later, he was referred to a pulmonologist for additional evaluation after presenting with decreased diaphragm excursion, which was causing a disordered breathing pattern. He was treated with medication and referred to additional rehabilitation for low back pain management after a thorough physical examination that also included checking the patient's core and deep breathing muscles Activities of Daily Living (ADL) activities.
